Important Tips for Medical Practitioners

• This is a Process, Not a Procedure: Patient counseling is paramount. Emphasize that Sculptra requires a series of treatments (typically 2-4 sessions spaced 4-6 weeks apart) and that final results manifest gradually over 3-6 months. It is an investment in collagen, not an instant filler.

• Master the Dilution & Reconstitution Protocol: For facial use, reconstitute each vial at least 24 hours (preferably 72 hours) before use with 9 mL of sterile water. Agitate gently. Before injection, add 2 mL of lidocaine to the 9 mL of product for a total of 11 mL. This extended dilution is non-negotiable to minimize the risk of papule formation.

• Precise Injection Technique is Critical: Inject deeply into the subcutaneous or supraperiosteal plane using a 25G or longer needle. Use a fanning or linear threading technique while depositing the product in small, even aliquots. Never inject intradermally or as a superficial bolus.

• Mandatory Post-Treatment Massage: To ensure even distribution of PLLA microspheres and prevent clumping, you must instruct the patient to massage the treated area for 5 minutes, 5 times a day, for 5 days (the "5-5-5" rule). Demonstrate the massage technique in-office.

• Management of Nodules: The most common adverse event is the formation of palpable, non-visible papules or nodules. This is most often caused by insufficient dilution, superficial injection, or inadequate massage. Most can be treated with massage, intralesional steroids, or hyaluronidase (for associated edema).
